DOL requirements for Application are Birth Certificate, Social Security Card, Valid Drivers License, Sealed High School Transcript or GED.
There is a $50 application Fee. Once accepted to the program the fee will be used toward the purchase of school books.
Applications are accepted Monday-Thursday 8am-3:30pm in person and must have documents to fill out application.
